What Is Vitamin D?
Vitamin D is a fat-soluble vitamin that the body needs to stay healthy. It comes from:
- Certain foods
- Dietary supplements
- Production in the skin when exposed to sunlight (UV light)
Vitamin D is made in the skin from cholesterol with the help of enzymes. Many people have low vitamin D levels, especially those who:
- Spend little time in the sun
- Live in northern climates
- Have darker skin tones
What Does Vitamin D Do?
Vitamin D also acts as a hormone, meaning it sends signals around the body through the bloodstream.
It works by attaching to the vitamin D receptor (VDR), which is found in many tissues, including bone, kidneys, and intestines.

Vitamin D helps:
- Regulate calcium and phosphate levels

How Do the Kidneys Activate Vitamin D?
Most vitamin D in the body is initially in an inactive form, called cholecalciferol (vitamin D₃). This inactive form cannot be used by the body.
Vitamin D must be converted into its active form, known as 1,25-dihydroxyvitamin D (calcitriol).
Two-Step Activation Process
- Liver – vitamin D is partially processed
- Kidneys – vitamin D is fully activated
The kidneys use an enzyme called CYP27B1 to complete this final activation step.
Because of this, the kidneys play a crucial role in vitamin D function.
What Happens in Chronic Kidney Disease (CKD)?
In chronic kidney disease, the kidneys are damaged and produce less CYP27B1. As a result:
- Less vitamin D is activated
- Blood calcium levels fall
- Bone strength is reduced over time
This can lead to a condition known as renal bone disease.
Vitamin D, Calcium, and Parathyroid Hormone (PTH)
Low calcium levels trigger the release of parathyroid hormone (PTH) from glands in the neck.
PTH increases blood calcium by:
- Activating osteoclasts
- Releasing calcium from bones
In CKD, ongoing low calcium and low active vitamin D cause high PTH levels. This further weakens bones and increases fracture risk.
Treating Vitamin D Problems in CKD
Many of the calcium and bone problems in CKD can be improved by:
- Vitamin D supplements (including activated forms)
- Correcting calcium and phosphate levels
- Reducing high PTH levels
Early treatment helps protect bone health and prevent complications.
