A group of doctors, hospitals, and providers coordinating care for a patient population; taking financial responsibility for quality outcomes and costs, often using a single budget or shared savings model, with the goal of integrated, patient-centered care.
They attempt to integrates and merge funding of primary and secondary care (and ideally social care); providing incentives to keep people healthy and out of hospital.
It was/is the basis for the development – in the NHS in England – of Integrated Care Systems (ICSs), now more commonly called Integrated Care Boards (ICBs).
